The Speech Language Pathologist (SLP) evaluates and assesses the patient's oral-motor, breathing, swallowing, voice fluency, speech and language skills. From this assessment, under the guidelines of a physician, the SLP treats the patient with a program developed specific to patient's function, diagnosis, and potential in an attempt to restore the patient to a functional level. Evaluate, treat, and provide discharge planning with team members for patients, specific to diagnosis in the areas of oral-motor, breathing, swallowing, voice fluency, speech and language skills, and coordinate with other team members. Provide education and training to patients and family with HEP, safety training, and education specific to diagnosis. Provide documentation of patient treatments, evaluations, and training to family/caregivers. Maintain a level of professionalism at all times and report unethical situations to the Board of Speech-Language Pathologists or appropriate personnel. Provide documentation of specific treatment/evaluation/progress according to established standards. Observes safety precautions in all aspects of job responsibilities. Maintains a clean and hazard-free environment. Participates in performance improvement activities. Participate in all mandatory case conferences Participate in staff development activities, which may include providing inservice training to other home health personnel; participate in inservice education programs.
